Each of the above package will get you a wonderfully custom designed banner and in any size you require (pls pick the right package/category in relation to your size). I will perform 2 rounds of edit (edits can take another day). 

So to begin, I need:

1. URL to your site if any.
2. Size in pixels or inches (width by height). 
   
3. Text to go on the banner
   
4. Format (PNG or JPG or PDF) *PSD source layered file cost another $10
5. Any logos, images or graphics you may have (I do provide royalty free stock images but there's also an option here to buy GettyImages® stock image)
